How the score works

What matters for healthcare

  1. Specialty acceptance

Each PEO decides which businesses it takes on. Ask early whether your specialty and staff mix qualify, and how workers' compensation is handled for clinical roles.

  2. Overtime for shift staff

Hospitals and residential care establishments may pay overtime on a fourteen-day period (after 8 hours a day and 80 in the period) with a prior agreement with staff. Ask whether the PEO's payroll supports it.

  3. License and credential tracking

Expired licenses create risk for the practice, not only for HR. Check whether the PEO's HR platform tracks licenses and expiry dates or connects to your system.

  4. Who is liable for payroll taxes

The IRS says a certified PEO (CPEO) is generally solely liable for the employment taxes on the wages it pays to worksite employees. Ask whether the PEO is IRS-certified and what stays with you in the agreement.

Frequently asked questions

Is a PEO for healthcare the same as PEO health insurance?

No. This page ranks PEOs that list healthcare among the industries they serve, for medical and dental practices and other care businesses. PEO health insurance usually means the group health plan a PEO offers to the employees of any client company. A healthcare practice that joins a PEO can get both: HR and payroll support for clinical staff and access to the PEO's benefits.

Can healthcare practices use a PEO?

Yes, if the PEO accepts their specialty: each PEO decides which businesses it takes on, and workers' compensation for clinical roles is part of that review. Ask each PEO about your specialty, headcount and states, and which payroll and HR duties stay with you under the agreement, before comparing prices.
